What we collect
Health goals you select, eligibility answers, state checks, order history, and device data that could show an interest in a health service.
If you start an online intake, we save your contact details, the treatment you selected and whether you completed the questions. Your detailed answers are not saved until our care team completes your consultation with you.
Why
To check eligibility, route your information to a licensed provider, process orders and shipping, give support, keep the site secure and meet legal duties.
Your rights
Ask what we hold and who we shared it with, ask us to delete it, or withdraw consent. Email support@blueonyxhealth.com with the subject "Consumer health data request". We reply within 45 days. You can appeal a decision by replying to it; Washington residents may then contact the Washington Attorney General.
